This template can be used to assist with requesting for Credit Disclosure information from the Credit Provider (CP). If you do not have the demand letters, you can request for a copy of this information (along with a transcript of all forms of communications you had with the CP) from the CP. As per the Australian Privacy Principle Chapter 12:12.1, Credit Providers (CP) are obliged to provide you with the information within 30 Days.

Dear Sir/Madam,

I have received a copy of my credit file and note that <Insert CP’s Name> have made an entry to my Equifax credit file <Insert PAS Number> on <Insert the Reported Date of the Default> I am disputing this entry and the debt, as I believe it is reported in error. I would genuinely appreciate if you could forward all notes, records, emails and written correspondence to me in relation to this matter as required by the Credit Reporting Code of Conduct 2.21. I draw your attention to the following Privacy (Credit Reporting) Code 2014 (Version 2) and Australian Privacy Principles Chapter 12 which indicates that I am within my rights to request such information:

Privacy (Credit Reporting) Code 2014 (Version 2) 2.21 – A credit provider must, when so requested in writing by an individual, attempt to give that individual access to any of his or her credit reports which are in the possession of the credit provider within 10 working days, and in any event, must give access within 30 calendar days of receipt of the individual’s request.

As per the OAIC, Australian Privacy Principle Chapter 12:12.1 An APP entity that holds personal information about an individual must, on request, give that individual access to the information
